Performance Profile
12 questions, then name, email and phone. The moment they submit, their answers and fit score are saved to your store — and they're sent to their results instantly.
Results Page
Shows their fit score, their recommended system, and either the $99 deposit button (which goes straight to Shopify checkout) or the waiting list signup.
Where Everything Lives in Shopify
Every Lead Becomes a Customer
Each quiz submission creates a customer record in your Shopify admin, automatically tagged with their fit score band and recommended system. You can filter, export, and email these lists anytime — they're yours, inside Shopify.
The $99 Deposit Is Real Shopify Checkout
The deposit is a $99 product in your store. When someone clicks "Reserve My Spot", they go through Shopify's own secure checkout — cards, Apple Pay, Google Pay, Shop Pay all work out of the box.
- Every deposit shows up as a normal Shopify order
- Payouts, refunds and receipts handled by Shopify
- At launch, the $99 is applied to their first order as a discount
You Can Edit Everything Yourself
All three pages are built as sections in your theme (Horizon — Shopify's newest theme). That means:
- Change headlines, prices, photos and button text visually
- Adjust quiz questions and answer wording without code
- Reorder or hide sections with drag and drop
Waiting List Included
Visitors who score below 60 — or who aren't ready to pay — join the waiting list with one click. They're tagged separately so you can send launch updates and early-access offers to exactly the right group.
